Virginia Department of Health (VDH) School Minimum Immunization Requirements

Documentary proof shall be provided of adequate age-appropriate immunization with the prescribed number of doses of vaccine indicated below for attendance at a public or private elementary, middle or secondary school, childcare center, nursery school, family day care home or developmental center.  Vaccines must be administered in accordance with and administered within the spacing and age requirements of the schedule of the Centers for Disease Control and Prevention, American Academy of Pediatrics (AAP), and American Academy of Family Physicians (AAFP).

Children vaccinated in accordance with either the current AAP or AAFP schedule or the AAP or AAFP catch-up schedules (including meeting all minimum age and interval requirements) are considered to be appropriately immunized for school attendance (see “Supplemental Guidance for School-Required Immunizations” for additional information).

Children entering daycare, public, and private schools are required to give proof of vaccination before enrolling in school. These clinics are for ALL preschool, Headstart, rising kindergarteners, 7th and 12th graders. 

Required Immunizations Include:

• Kindergarten: DTaP, Polio, MMR, Varicella, HepA (1st dose followed by 2nd dose in 6 months).
• 7th Grade: HPV, MenACWY, Tdap
• 12th Grade: MenACWY

IMPORTANT:

• A parent or legal guardian MUST be present at the time of service if the child to be vaccinated is under the age of 18.
• If insured, bring an insurance card at the time of service.
• If you or a family member in the household is sick on the day of service or have any fevers in the last 24 hours, please do not show up to the clinic on that day.
• If you have received vaccines in another state other than Virginia, please bring a copy of your record with you.
